Title: Program Manager – Community Public Health Team (CPHT)

Hours: Full-time, occasional evenings, and weekend work possible
Starting wage:  $68,640 annually 

CPHTs will consist of coordinated, place-based, and community-driven health practitioners who will work together with community members to identify local health priorities, reduce gaps in health outcomes, and improve the conditions essential for overall health and well-being in high-need communities throughout Los Angeles County. CPHTs will use a shared leadership model; each CPHT will consist of staff from Community-Based Organizations, Health Care Partners/Federally Qualified Health Centers, and dedicated existing staff from LACDPH who will work together to provide targeted outreach, engagement, and services in select communities with the shared goal of improving health and well-being.

Experience & Requirements:

• Master’s degree in public health, Public Policy, Social Work, or other relevant degree AND a minimum of one (1) year of experience implementing public health or social service programs in underserved communities OR bachelor’s degree in a similar field and a minimum of three (3) years of experience managing grant-funded public health, social service and/or similar programs.
• Must have experience providing programmatic oversight (e.g., program operations, monitoring and tracking, reporting), assessment and evaluation, and program implementation in underserved communities, with minimum of one (1) year administering community engagement programs.

• Must have previous experience in working with underserved communities in the Antelope Vally region. 
• Bilingual preferred but not required. 
• Maintain a valid California driver’s license.
• Must have automobile and maintain automobile insurance.
• Meet basic health and background check requirements and physical job standards.

Essential Duties and Responsibilities:

• Responsible for overall program management, planning, execution of the process objectives, program deliverables, reporting, supervising the day-to-day activities of staff, and coordinating with secondary recipients and community members and acts as the liaison to Public Health for programmatic matters.
• Responsible for reporting on programmatic activities, data metrics, and project outcomes. 
• Compile CPHT monthly and final progress reports in addition to year-end reports and regular invoices for submission to Rising Communities and LACDPH administrative staff.
• Acts as a liaison and programmatic lead and point of contact between Rising Communities and LADPH for programmatic matters.
  • They will be responsible for ensuring that contracted partners are aware of all project requirements and participate in meetings to provide guidance to staff between Bartz Altadonna and AVPH. 
• Participate in regular meetings with Rising Communities and/or LACDPH administrative staff to check in on project progress.
• Participate and facilitate in weekly/quarterly planning meetings with members of the CPHT network.  
• Must work in partnership with Administrative Program Manager to ensure execution of process and program deliverables.
• Develop and meet with a CPHT Advisory Committee to allow community members and partner agencies opportunities to provide guidance to CPHTs through ongoing participation. Assist in developing a community driven action plan with pivotal community member input to meet community needs. 

• Lead efforts in developing a written internal governance structure outlining, defining the roles and responsibilities of each partner, identifying the process of administrative decision making and identifying process of programmatic changes. 
• Organize and executive at least 2 community convenings a year.
• Oversee CPHT Advisory Committee, inclusive of community leaders to allow community members to participate in decision-making. 
• Stay abreast of current and emerging health issues to assist with educating teams and informing the local community response.
• Ensure that program staff participate in centralized trainings, including orientation and refresher trainings, and provide guidance to staff to reinforce messaging and field guidelines, as coordinated by Rising Communities and/or LACDPH administrative staff.
• Special projects and other administrative duties as assigned.

Working Conditions:

• Primarily an in the field and office environment; with conditional hybrid – remote option.
• Employee will be required to sit, perform repetitive physical and mental activities, have face-to-face and telephone contact with individuals, work with others in a group or team, and work with external customers or the public. 
• May be required to lift and/or move up to 25 pounds.
• Work may be performed under stressful and emotional conditions.
• Must be fully vaccinated for COVID-19
